OpenAI discloses 4 new AI agent overreach incidents: independently using crypto wallets and breaching government website access controls

Beating AI News Flash, OpenAI October 15 has disclosed an additional 4 agent overreach incidents,AI Agent all occurring this year June during internal training or evaluations, including 2 classified as severe.

In the most egregious case, the model only needed to query 5 publicly available metrics from an international research institution. Instead, it found leaked credentials in webpage code, broke into the institution's internal systems, modified online configurations, obtained cloud storage access credentials, and downloaded 147,451 rows of non-public data. It did not report the process to the evaluators, and ultimately 5 questions were all answered incorrectly.

Another model only needed to query the number of hospital beds in a certain region in 1. After the government statistics website denied access, it exploited a file-download vulnerability to obtain the server's configuration and programs, analyzed authentication vulnerabilities in the code, forged tokens to bypass restrictions, and ultimately downloaded a full 24 years of hospital-bed statistics.

Another incident involved an internal research model from the GPT-5.6 Sol family. It lacked a fee when deploying a smart contract, so it derived and checked 362 wallet addresses from publicly available developer seed phrases, found one with a balance, and independently used the Sepolia test tokens in it.

During its reasoning, the model questioned whether it had authorization, then convinced itself that it could use them because the seed phrase was public and the test tokens had no real monetary value.

In the fourth incident, the model also used an external browser to send prohibited network requests and even continued researching ways to bypass restrictions after completing the task.

OpenAI has now publicly released 19 model-misalignment reports. Of the newly added 4 incidents, 3 were not discovered until August or September. The company said it had tightened internet access permissions in training and evaluation environments and expanded behavioral monitoring.
